Please note this is an event for sisters between 12 and 18 only.

Tickets purchased for children not meeting the age criteria will be cancelled.

1. I agree to myself / my child taking part in the Laser Quest Event.
2. I confirm to the best of my knowledge that I / my child does not suffer from any medical condition other than those listed above.
3. I acknowledge that the activity is of a physical nature and confirm that I have the physical ability to partake in such an activity
4. I consent to myself / my child travelling by any form of public transport, minibus or motor vehicle driven by an appointed volunteer or any other parent attending.
5. I understand that the organisers accept no responsibility for loss, damage or injury caused by, travel to/from, or during attendance of the activity except where such loss, damage or injury can be shown to result directly from the negligence of the organisers.
